Below, you will find various request forms for services offered by the City of Dorval.

For forms that require the signature of a commissioner of oath, the applicable fees are as follows:

  • Any oath of 3 documents or less: $5.
  • Any oath of 4 to 7 documents: $10.
  • Any oath of 8 documents or more: $15.

Privacy Policy and Protection of Personal Information
The information collected via these forms is required to process the various services offered by the City of Dorval, including claims. It is intended to be processed exclusively by City of Dorval employees who require access to it in the course of their duties, as well as by current or future subcontractors or partners for exclusively technical and logistical reasons. Physical, organizational, contractual, and technological security measures have been put in place to protect it. We will use, disclose, or retain these records only as long as necessary for the fulfillment of those purposes and to the extent permitted or required by law. You have the right to access and rectify your personal information, and you may withdraw your consent at any time. Certificat of Residency

This document confirms the address of a person. The person who wishes to obtain this certificate must bring a letter or the attached form to our commissioner for oaths. The applicant must show two proofs of residence such as a tax bill, lease, public utility bill (except a telephone bill) or driver's licence and two signed pieces of identification, at least one of which has a photo (Medicare card, driver's licence, passport, citizenship card, permanent resident card or social insurance card).

Claims

Anyone who believes that they have suffered material damages which they intend to claim against the City must, within FIFTEEN days of the incident, send a written notice or the duly completed attached form. To avoid delays, please send your form or notice to City Hall offices by certified mail with all documents necessary for us to study your file (i.e. bills, estimates, photos, reports).

Similarly, it is strongly recommended that individuals, who as a result of an accident suffered bodily injuries for which they plan to claim damages and interest from the City, submit a written notice as quickly as possible.

The notice must be written by the claimant and must include the following information:

  • last and first name of the claimant;
  • address of the claimant;
  • cause of the damages;
  • date and location of the incident.

Consent Letter for Children Travelling Abroad

It is strongly recommend that Canadian children carry a consent letter if they are travelling abroad alone, with only one parent/guardian, with friends or relatives or with a group. For the purposes of this consent letter, a Canadian child is defined as anyone who is under the age of majority.

Financial Aid - Reusable Sanitary Products

The City of Dorval now offers a financial aid program aiming to promote the use of various reusable sanitary products. For details on the program, click here.

Any request for financial aid must be made in writing on the form prescribed for this purpose and filed with the Mayor’s office no later than three months after the acquisition of the product.

Financial Assistance - Quick Charging Station for Domestic Use

The City of Dorval encourages the development and the use of greener vehicles, including electric vehicles. A financial assistance program was put in place for the purchase and installation of a quick charging station for domestic use. For details on this program, click here.

To submit a request, bring your documents to the Urban Planning Department (60 Martin Avenue) or send them by email to amenagementurbain@ville.dorval.qc.ca.

In its commitment to improving the quality of housing in our municipality, the City of Dorval adopted a new by-law to implement the Rénovation Québec program for residences with cracked foundations.

The Fund to Assist and Support Dorval Citizens’ mission is to help Dorval residents of all ages in order to encourage their personal accomplishments in regards to culture, arts, sports, and leisure. A second component of the Fund also allows for support towards the development of the City’s parks. For details on this program, click here.

A general declaration is required when a person must swear under oath or make a solemn declaration.

The person writes a declaration or completes one of the following forms. The document MUST be signed before a commissioner for oaths.

It is important to mention that the commissioner for oaths does not approve the content of the document but only the authenticity of the signature.

In order to improve the speed and efficiency of our response during disaster situations (heat waves, ice storm, etc.), we are inviting all residents who may require special assistance, such as seniors and persons living alone or with any disabilities, to register with the Community Aid Service of the City of Dorval.

To register, print the form, complete it, and forward it to the address indicated thereon. All information provided will remain strictly confidential and will be used to intervene in disaster situations only.

Life Certificate

A life certificate is required for persons who receive an income from a foreign country since the organization that deposits the income insists on proof that the recipient is still alive. The recipient generally receives a form but the attached form can also be used. The form MUST be signed at our offices before one of our commissioners for oaths.

Public Domain Occupancy Request

This form is required for any request for a permit to obstruct or occupy the public domain in Dorval. All relevant documentation (plan, prior application, consent, etc.) must be attached to this form for processing. Allow five (5) working days for the application to be processed.

Refund for a course or activity

This form is to be filled out by those wishing to obtain a refund for a course or activity for which they have registered. The refund policy is the following:

Only written requests, addressed to the Leisure and Culture Department, will be considered. A prorated reimbursement will be allocated before the fourth class. Administration fees of $11.50 will be withheld.

For a refund for medical reasons, the individual must present a doctor’s note attesting to his/her medical condition.

For all refunds, please allow four to six weeks for a check to be issued or a credit on the account to be made.

Request for Financial Assistance - Emerald Ash Borer

As part of its fight against the spread of the emerald ash borer, the City of Dorval has established a financial assistance program to assist its citizens in the costs incurred for the treatment and cutting down of ash trees located on their property. For details on the program, click here.

Request for Municipal Services

This form is required for any request to connect or abandon services to the municipal networks (drinking water, waste water, driveway access). It is also required during the construction or replacement of the private part of the service connection as part of the issuance of the certificate of authorization to connect the private part to the public part.

Subsidy for the Replacement of Wood-Burning Appliances

The City of Dorval is now offering financial assistance to help them replace their current appliance with one that respects environmental standards. This program is designed for houses only (single-family, duplexes or triplexes, multi-family or a residential part of a mixed-use building). For details, click here.

Water Retention Form

This form is required with any permit request for construction/redevelopment projects for which the waterproof area is of 1000 square metres or more. It was developed to facilitate the validation of the compliance of stormwater retention work for projects subject to stormwater retention regulations.

As part of its fight against the spread of the emerald ash borer, the City of Dorval has established a financial assistance program to assist its citizens in the costs incurred for the treatment and cutting down of ash trees located on their property. For details on the program, click here.

To submit a request for financial assistance, you must fill out the request form and email it to greenline@ville.dorval.qc.ca.

Request for Municipal Services

This form is required for any request to connect or abandon services to the municipal networks (drinking water, waste water, driveway access). It is also required during the construction or replacement of the private part of the service connection as part of the issuance of the certificate of authorization to connect the private part to the public part.

Subsidy for the Replacement of Wood-Burning Appliances

The City of Dorval is now offering financial assistance to help them replace their current appliance with one that respects environmental standards. This program is designed for houses only (single-family, duplexes or triplexes, multi-family or a residential part of a mixed-use building). For details, click here.

Water Retention Form

This form is required with any permit request for construction/redevelopment projects for which the waterproof area is of 1000 square metres or more. It was developed to facilitate the validation of the compliance of stormwater retention work for projects subject to stormwater retention regulations.
