An oaths service is offered at the reception of City Hall for citizens who need to sign a document before a commissioner of oaths, such as an affidavit, a life certificate, an authorization to travel with a minor child, a certificate of lodging or a letter of invitation.
The service is available Monday to Friday from 8:30 a.m. to noon and from 1 p.m. to 4:30 p.m. The cost is $5 for 3 documents, $10 for 4 to 8 documents, and $15 for more than 8 documents, all according to the By-law respecting fees for municipal services in effect for the current year.
Any person wishing to obtain an administration of oath service must present themselves at City Hall with a recognized piece of photo identification and the completed but unsigned form. The form must be signed in front of the Commissioner of Oaths.

The Commissioner of Oaths may refuse to administer an oath if the document is not done in the required form, if it is visibly erroneous or if the declarant does not appear to be in possession of all his faculties or able to express his will.
This document confirms the address of a person. The person who wishes to obtain this certificate must bring a letter or the attached form to our commissioner for oaths. The applicant must show two proofs of residence such as a tax bill, lease, public utility bill (except a telephone bill) or driver's licence and two signed pieces of identification, at least one of which has a photo (Medicare card, driver's licence, passport, citizenship card, permanent resident card or social insurance card).

A life certificate is required for persons who receive an income from a foreign country since the organization that deposits the income insists on proof that the recipient is still alive. It is strongly recommend that Canadian children carry a consent letter if they are travelling abroad alone, with only one parent/guardian, with friends or relatives or with a group. For the purposes of this consent letter, a Canadian child is defined as anyone who is under the age of majority.

A general declaration is required when a person must swear under oath or make a solemn declaration.
The person writes a declaration or completes one of the following forms. The document MUST be signed before a commissioner for oaths.
